For all expenses claimed, please fill out the form below. The expense claim form can be filled out on your computer, but all documents (including a printed copy of the form and all related receipts) must be mailed to the office where your Labour Relations Officer is located.
Please note the glitch from the extra $13.00 added to the expense claim form has been corrected. Please delete all old copies of the form and erase your cookies when using the new form. If you notice any other glitches, please report them to the Communications Officer.
